Intake Support Representative
Roles & Responsibilities • Verify and validate referral packet information to ensure accuracy before the first visit • Enter referral data promptly and accurately into the agency systems • Follow up with referral sources, clinicians, and external partners to resolve missing or inconsistent information • Prioritize and manage referrals to meet timelines and maintain quality patient care • Demonstrate dependable attendance and maintain required schedule commitments • Complete required onboarding screenings (CORI/SIR) and ensure results are on file before starting assignment • Communicate professionally with patients, families, and internal teams to support smooth intake processes • Problem-solve independently and follow through on solutions to reduce delays and improve workflows
Qualifications • 1+ year full-time experience in a medical office (required) • 1+ year full-time data entry experience (required) • High school diploma (required) • Working knowledge of health insurance coverage requirements (required) • Knowledge of medical terminology (required) • General computer skills and comfort with data entry systems (required) • Active/current driver’s license and access to a personal vehicle (required for travelers) • Permanent address included in profile and any requested time-off (RTO) documented at submission • Home Health Agency experience (highly preferred) • Supervisory experience (preferred)
Education • High school diploma
Knowledge & Skills • Data entry accuracy and speed • Familiarity with medical terminology • Understanding of health insurance coverage basics • Strong time management and ability to prioritize • Ability to work independently with minimal supervision • Problem-solving and follow-through skills • Dependability and punctuality • Teamwork and strong interpersonal communication • Attention to detail and ability to work through interruptions
