Roles & Responsibilities • Provide compassionate, hands-on care to residents — assisting with activities of daily living such as bathing, dressing, grooming and toileting • Take and record vital signs accurately and report any changes to the nursing team • Assist with safe transfers and mobility support using proper body mechanics • Document patient interactions, care provided, and any observed changes clearly and promptly • Maintain infection control and safety practices to protect residents and staff • Communicate effectively with nurses, therapists and family members to support coordinated care
Qualifications • Active state Certified Nursing Assistant (CNA) certification • Current CPR/BLS certification preferred or required by facility • Comfortable assisting with ADLs and basic patient care tasks • Strong interpersonal skills and a caring, patient-centered approach • Ability to follow care plans and accurately document clinical information
Education • High school diploma or GED and completion of an accredited CNA training program
Knowledge & Skills • Proven ability to perform patient transfers and mobility assistance safely • Competence in taking vital signs and observing/reporting changes • Familiarity with infection control and standard safety procedures • Clear verbal and written communication skills for documentation and teamwork • Basic comfort with electronic charting or willingness to learn EMR systems
