POSITION PURPOSE: The Occupational Therapist is responsible for patient evaluation, development and follow through of treatment plans to support the mission of Salem Health.
Education: BS, MS, or Doctorate in Occupational Therapy, or certification by NBCOT required.
Experience: Clinical experience in the practice setting is required
Licenses/ Certification/ Registration 1. Graduate of a program approved by the Committee on Accreditation in Education of the American Occupational Therapy Association, and holds a valid license to practice in the state of Oregon or is license eligible. License must be verified prior to start date. 2. BLS from the AHA within 90 days of hire required.

Travel Compliance Required at time of Submittal: Current profile document with finalized RTO and a minimum of one supervisor reference is required. All certifications listed in job description and a copy of the Nursys/CNA license (PDF format, dated within last 30 days) must be provided even if OR license is not currently in hand. Epic Charting experience is a hard stop requirement. **For Allied positions, a specialty-relevant certification must be sent at the time of submission.
Licensure: Oregon RN license is NOT required at time of submission pending is okay. Oregon CNA license must be in hand at time of submission. No pending Oregon CNA -or- expired Oregon RN/CNA licensure will be accepted under any circumstance. *Any license/background hits require a detailed written statement from the traveler and will be reviewed on a case-by-case basis. Travelers with license or background hits are ineligible for 90-day temp license waiver.
Start/ RTO / Schedule Policy: Facility prefers to only book 3-4 weeks out from current date but check job descriptions for specific start date requirements. RTO must be finalized at time of submittal. Additional RTO requests after submittal may result in removal from consideration or contract cancellation. Facility does not accommodate block scheduling.
Candidates may be submitted to one job within the Salem health system, with an email detailing other units they are interested in at the time of initial submittal. Travelers will not be submitted to additional units. Candidates may float within hospital or to an affiliate hospital nearby - Travelers are first to float to West Valley Hospital, located approximately 15 minutes away from Salem Hospital. Refusing to float may result in contract cancellation.
Return Staff Policy: Former permanent employees may not submit to travel positions at any Salem Health Hospitals and Clinics for six months after their employment termination date.
