Client Notes: Barton Health is a non-profit, community health system situated on the south shore of beautiful Lake Tahoe nearly 6,200 feet above sea level. Since opening in 1963, Barton has expanded its services to serve both its residents and visitors to Lake Tahoe. Barton Health includes a community hospital and Level III Trauma Center including 63 licensed acute care beds and 48 licensed long term beds.
Submittal Qualifiers: • Does candidate have an active BLS issued through American Heart Association (AHA)? • Does candidate have current or prior discipline on their professional license or certification? Requirements: • Resume Must have candidates current name, no nicknames, or abbreviations. Work history must include name of facility worked, city and state, title, department, and dates employed from the last 7 years. Agency names are not accepted as the employer. Highest level of education listed, including month/year of graduation and degree achieved if applicable. • Skills Checklist Completed within one (1) year of submittal for specialty where candidate is submitted • Two (2) References Two (2) professional references from work history occurring within the past three (3) years. One must be from a supervisor (charge nurses are considered supervisors). Reference must include the full name (first & last name) of the person providing reference. The name of the facility and the dates of employment must match the resume. Reference must include a breakdown of skills and strengths with a rating score. Letters of recommendation will not be accepted. • Online verification of professional license or certification Verified within 30 days of submission. Nursys acceptable. • Verification of California Professional License Online verification of professional license completed within thirty (30) days of start and at time of extension. For RN/LPN: verification must also include a Nursys report- uploaded together. • Drug Screen 9-panel urine drug screen collected within thirty (30) days of start date and must include the following drug panels: Amphetamines, Barbiturates, Benzodiazepines, Cocaine, Methadone, Methaqualone, Opiates, Phencyclidine, Propoxyphene. **Do not test for THC** Client will not accept drug screens that include THC or where document has been altered to remove THC. Client will perform an oral swab for THC metabolites on the first day of assignment. If the results positive, assignment is terminated immediately for cause and no penalty will be assessed to Client. • MMR MMR shown by proof of two (2) MMR vaccinations or positive IGG titers. Decline form accepted for low/equivocal titers, Affiliate form acceptable. • Varicella Varicella shown by proof of two (2) vaccinations, positive IGG titer, or provider verified history. Decline form accepted for low/equivocal titers, Affiliate form acceptable. • Hepatitis B Hepatitis B shown by proof of vaccination series, positive surface antibody titer or declination • Flu Influenza vaccination for Assignments occurring between 10/1-3/31 or Flu Shot Declination. Affiliate declination form is acceptable. • COVID For no previous vaccination history, please upload a decline form. For vaccination(s) given before September 2022, please upload proof of vaccination and decline form to decline subsequent vaccinations. For vaccination(s) given after October 2022, proof of vaccine is all that is required. • TDAP Tdap administered with ten (10) years of start date or affiliate decline form accepted. • TB Test Documentation of TB skin test, TB QuantiFERON, or T-Spot completed within one (1) year prior to the start date, and required annually. A TB screening form is required for those known to test positive for TB upon hire, and annually thereafter. Upon hire, known positives must have had a Chest X-Ray within the last year. A Chest X-Ray is not required annually and can be completed every 5 years. • Respiratory Fit Test Respiratory Fit Test Required within 12 months of start and required annually. Acceptable masks include: 3M N95 1870 Plus, 3M N95 1860s , 3M N95 1860, N95 Gearson 1730, N95 ProGear Regular RP88020, N95 ProGear Small RP88010 • Background Check Background Check completed thirty (30) days prior to start date. National Criminal Check, FACIS Level Ill, National Sex Offender, all counties lived and worked within past 7 years, including for any alias. SSN Address trace is required. • OIG (Office of Inspector General) OIG completed within thirty (30) days of start date, and at time of extension. Must be run for all alias names that appear on the SSN address trace. • SAM (System for Award Management) SAM completed within thirty (30) days of start date, and at time of extension. Must be run for all alias names that appear on the SSN address trace. • Specialty Competency Exam Competency Exam completed within one (1) year of start date for the specialty corresponding to the CP s Assignment; required annually. An 80% or passing rate is required. • Joint Commission - Core I, II, and III Training Joint Commission Core I, ll and III completed within a year of start; required annually • Government ID Copy of valid Driver's License or government issued ID • Results - THC Oral Swab Client will provide results of THC oral swab performed on first day • CHT - Certified Hand Therapist Certification PREFERRED

Ideal if person had some Acute care experience too National Board Certified Occupational Therapist (NBCOT) Certification-Required.
